髋臼双柱骨折合并后壁骨折的手术治疗

Surgical treatment of acetabular double columns associated with posterior wall fractures

目的探讨髋臼双柱合并后壁骨折的临床特点和手术疗效。方法对29例髋臼双柱合并后壁骨折采用双切121分步入路切开复位，重建钢板内固定。结果按Matta标准，解剖复位21例，满意复位5例，不满意复位3例。随访13～38个月，平均19个月，关节功能按d’Aubinge-Possel评分系统评定，优17例，良6例，可3例，差3例。结论双切口分步入路，重建钢板固定是治疗髋臼双柱合并后壁骨折的有效方法。

Objective To explore the clinical characters and surgical treatment of acetabular double columns associated with posterior wall fractures. Methods 29 cases were enrolled. Open reduction and reconstruction plates fixation were performed for acetabular double columns associated with posterior wall fractures by combined anterior and posterior approaches. Results According to Matta reduction system, the reduction was anatomical in 21cases; good in 5 and fair in 3. 29 cases were followed up for 13 -38 months, average 19 months. According to modified d＇Aubigne- Postel hip function scoring system, the final results were evaluated as excellent in 17 cases, good in 6, fair in 3 and poor in 3. Conclusions It is effective to treat acetabular double columns associated with posterior wall fractures with open reduction and reconstruction plates fixation by combined anterior and posterior approach.
