高频振荡通气 被引量:3

摘要 传统的容量限制通气即低潮气量(TV)肺保护策略[TV≤6ml/kg,吸气平台压≤30cmH2O(1cm H2O=0.098mmHg)]与更高的TV(12ml/kg)策略比较,能降低成人急性呼吸窘迫综合征(ARDS)患者的病死率。大量的随机对照实验证明,使用TV限制肺泡过度扩张的通气策略也有助于降低病死率。然而实际上许多呼吸窘迫综合征患儿使用肺保护策略[吸入氧浓度(FiO2)≤0.60,吸气平台压≤30cmH2O]并不能达到理想的气体交换目标,病死率仍然较高。随着高频振荡通气(HFOV)在新生儿和儿童急性呼吸窘迫综合征中的成功应用,已激起了广大医务工作者浓厚的兴趣。现将HFOV气体交换特点、通气策略和实施、临床应用及并发症等方面作一综述。
