冠心病患者血抵抗素与胰岛素抵抗及冠脉狭窄程度的相关性研究 被引量:2

Relationship of Serum Resistin with Insulin Resistance and Degree of Coronary Stenosis in the Patients with Coronary Heart Disease

摘要 目的研究冠心病患者中抵抗素与胰岛素抵抗及动脉粥样硬化的关系。方法根据冠状动脉造影检查结果将57个受试者分为试验组45例(狭窄程度≥50%)和对照组12例(狭窄程度<50%)。测量入选对象的空腹血清抵抗素、胰岛素浓度、胰岛素敏感性指数及冠状动脉狭窄程度评分,然后进行评价。结果冠心病患者空腹血清抵抗素浓度与空腹胰岛素及冠状动脉狭窄程度评分呈显著正相关[r分别为0.303(P<0.05),0.38(P=0.01)];血清抵抗素浓度与胰岛素敏感性指数呈显著负相关(r=-0.296,P<0.05);抵抗素浓度是影响胰岛素浓度的独立因素(R2=0.120,P<0.05);同时,抵抗素浓度是影响冠状动脉狭窄评分的独立因素(R2=0.224,P=0.01)。结论①冠心病患者空腹血清抵抗素浓度与胰岛素抵抗有显著正相关。②血清抵抗素浓度是影响冠状动脉狭窄程度独立影响因素,并与之呈显著正相关。 Objective To study the relationship of serum resistin(RSTN) with insulin resistance and degree of coronary stenosis in the patients with coronary heart disease(CHD). Methods According to the result of coronary arteriongraphy, the 57 subjects with CHD were divided into the experimental group (the degree of stenosis≥50 %, n= 45)and control group( the degree of stenosis〈50%, n= 12 ). Fasting serum RSTN, fasting serum insulin ( FSI), insulin sensitivity index(ISI) and score of coronary stenosis were determined in all the subjects. Results The serum RSTN was a significantly positively correlated with the level of FSI (r=-0. 303, P 〈0.05) and score of coronary stenosis (r=0.38, P =0.01) in the patients with CHD. There was a significantly negative correlation between serum RSTN and ISI (r=-0. 296, P 〈0.05). The serum RSTN was a independent factor influencing coronary stenosis score( R^2 =0. 224, P =0.01)and FSI level( R^2=0. 120, P 〈0. 05). Conclusions (1)There is a significantly positive correlation between serum RSTN and Insulin resistance in the patients with CHD. (2)The serum RSTN is a independent factor influencing degree of coronary stenosis and positively related to the degree of coronary stenosis.
