腰椎间盘突出再手术的原因和治疗探讨

Investigating the Cause and Treatment of the Re-operation of Lumbar Disk Herniation

目的:探讨腰椎间盘突出再手术的发病机制及临床特点.方法:对27例腰椎间盘突出症突出髓核切除术后疗效不佳或再次出现症状者进行临床分析及再次手术治疗.结果:术后对27例病人进行随访,优,44.4%;良,33.4%;可,14.8%;差,7.4%.结论:腰椎间盘初次手术前准确定位,术中操作仔细,彻底切除突出的髓核组织,但不要过多地切除正常的间盘组织及避免伤及硬脊膜,术后彻底止血及脊管内放置引流,为维持脊柱的稳定性,必要时可行脊柱内固定等措施都是提高手术成功率和防止腰椎间盘再突出的有效方法.

Objective： To investigate the disease mechanism and dinical characterstics of the re-operation to 27 patients, who suffered from lumbar disk herniation, the post - operation clinical effects of nucleus poolpous＇ resection were excellent in 44.4% ,good in 33.4% ,fair in 14.8% ,and poor in 7.4%. Conclusion：Before initial operation of lumbar disk herniation, we should locate exactly,operate carefully dintra - operation and resect the nucleus polpous tissue completely, but not over - resect the normal tissue and avoid hurting hydatid spinal cysts,control bleeding completely and place the drainage in spinal vessel post - operation in order to maintain the stability of spine, if necessary, it might be internal fixation,above all measures were effident to promote the successful rate of surgery and prevern the lumbar disk herniation again.
