留置导尿致尿路感染与集尿袋更换时间的相关性研究

Relationship Between Urinary Tract Infection due to Indwelling Urinary Catheterization and Urine Collection Bags′ Interval of Change

目的研究留置尿管患者两种不同集尿袋对尿路感染的相关性,以探讨两种集尿袋的更换时间。方法将留置导尿的女性患者65例,按单纯随机法分双号组32例与单号组33例,分别应用单向活瓣集尿袋与一次性塑料普通无菌集尿袋;通过定时做尿培养,观察不同留置导尿时间尿路感染的发生率。结果双号组尿路感染递增数3 d与57、d比较差异无统计学意义(P>0.05),3 d与9 d比较差异有统计学意义(P<0.05);单号组3 d与5 d比较差异无统计学意义(P>0.05),3 d与7、9 d比较差异均有统计学意义(P<0.05或P<0.01),两组患者不同留置时间3、57、d和9 d尿路感染发生例数结果比较P<0.05或P<0.01。结论单向活瓣集尿袋的更换时间以1次/周较适宜,一次性塑料普通无菌集尿袋的更换时间,以2次/周较适宜,不同集尿袋对尿路感染有不同影响。

OBJECTIVE To investigate factors contributing to the urinary tract infection arising from two kinds of urine collection bags and indwelling urinary catheterization in patients, to assess the urinary collection bag＇s interval of changing. METHODS Sixty five female patients with indwelling urinary catheterization were randomized into two groups： patients in even number group （n = 32） were provided with one-way urine collection bags, and those in the odd number group （n=33） were provided with general aseptic bags. The urinary tract infection rate was observed based on the results of regular urine culture at different times during the indwelling. RESULTS In the even number group, the urinary tract infection rate on d3 in comparison with that on d5 and d7 had no significant difference （P〉0.05） ; the difference on d3 with that on d9 was significant （P〈0.05） ; while in the odd number group, the difference on d3 with that on d5 was not significant （P〉0.05） ; the difference on d3 with that on d7 and d9 was significant （P〈0. 05 or P〈0. 01）, the results of comparing the cases with urinary tract infection indwelling on d3,5,7 ,and 9 in the two groups were P（0.05 or P（0.01. CONCLUSIONS It is better to change the one-way urine collection bags once a week, as for the general aseptic bags, twice a week. Different urine collection bags contribute different influence on the urinary tract infection.
