75例小儿先心病介入封堵术的麻醉分析 被引量:2

摘要 目的:探讨小儿先心病的麻醉处理方法。方法:75例ASAⅠ~Ⅱ级的先心病患儿静注东莨菪碱0.004mg/kg^0.008mg/kg或硫酸阿托品0.01mg/kg^0.02mg/kg,丙泊酚1mg/(kg.h)~2mg/(kg.h),肌注咪唑安定0.1mg/kg,盐酸氯胺酮1mg/kg^1.5mg/kg,并予鼻导管或面罩吸氧。术后入住麻醉苏醒室观察至患儿完全清醒。结果:全组平均手术时间(74.32±25.58)min。全部先心病患儿动脉导管未闭、房间隔缺损和室间隔缺损介入封堵均获成功。术中氯胺酮总量为(2.81±0.93)mg/(kg.h),丙泊酚总量为(5.23±2.20)mg/(kg.h)。74例患儿在全凭静脉麻醉下完成手术,占98.77%,所有患儿无麻醉并发症发生。结论:麻醉处理关键在于保持呼吸道通畅,减少呼吸抑制的发生。咪唑安定、氯胺酮、丙泊酚联合用药全凭静脉麻醉是安全、有效的麻醉选择。气管内插管麻醉在必要时可提供更安全和良好的手术条件。
