Application of 24h-AEEG in the forensic psychiatry appraisment for the criminal with doubtful epilepsy

目的初步探讨动态脑电图(AEEG)在可疑癫痫者司法精神医学鉴定中的评估价值。方法应用MB8500、FD8800动态监测技术,对39例可疑癫痫犯罪人的24小时AEEG进行跟踪监测。结果39例中,痫样放电12例(30.8%),非特异性异常7例(17.9%),总异常率为48.7%;痫样放电出现清醒期4例(33.3%),睡眠期出现8例(66.7%),清醒期与睡眠期之间有显著性差异(P<0.01);部位多见于颞区、额区和枕区局限性异常。结论AEEG对司法精神医学鉴定中可疑癫痫犯罪对象的评估提供重要的依据,但不能作为责任能力的判定依据。

Objective： To study the valuation of 24h ambulatory electroencephalogram（AEEG） applied in forensic assessment of doubtful epilepsy. Methods： A retrospective analysis of 2Ah - AEEG was done in 39 criminal with doubtful epilepsy recommended by judicial organization. Results： In 39 criminal with doubtful epilepsy examined with 24h- AEEG, there were epileptiform discharges in 12cases （30.8%）, nonspecific abnormal AEEG was noted in 7 criminal with doubtful epilepsy. The abnormal rate of AEEG was 48.7%. The time of epileptiform discharge was mainly in sleeping stage （66.7%）, and less often in the waking stage （33. 3% ）. There was a significant difference between two stage （ P 〈 0.01 ）. The local abnormal discharge was mostly seen on the frontal and temporal lobes. Condusion：AEEG could play an important role in the diagnosis of epilepsy, but it is not the determining evidence for the criminal responsibility judgment.
