摘要目的研究HBV前S1抗原检测的临床价值,即HBVPreS1抗原与具有病毒活动性复制意义的指标HBeAg和HBV-DNA之间的相关性其与肝功能指标ALT之间的关系。方法HBV前S1抗原和血清标志物(HBsAg、HBsAb、HBeAg、HBeAb、HBcAb-IgG、HBcAb-IgM)采用ELISA法检测;肝功能(ALT)用酶学速率法检测;HBV DNA用荧光定量PCR法检测。结果①HBeAg阳性组其HBV PreS1抗原检出率高。②HBV PreS1抗原与具有病毒活动性复制意义的指标HBV-DNA有良好的一致性。③慢性活动性肝炎肝功能指标ALT>40 u/l组其HBV PreS1抗原检出率高。结论HBVPreS1抗原和HBV DNA的检测结果可以较好地反映HBV的复制及慢性肝炎的活动情况。PreS1抗原可以作为HBeAg和HBV-DNA的补充,也可作为判断慢性乙型肝炎患者病情活动性的一项指标。对乙肝的诊断、疗效判断与观察具有较高的价值。 Objective To evaluate the relationship between HBV ProS1 antigen and the virus replication index（HBeAg and HBV-DNA） and liver function. Methods HBV preS1 antigen and serum marker （HBsAg, HBsAb, HBeAg,HBeAb, HBcAb-IgG, HBcAb-IgM） were detected by ELISA; Liver fuction（ALT） was detected by kinetic method; HBV DNA was deteced by fluorescent quantitation PCR.Results ①The detection rate of HBV preS1 antigen were high in HBeAg positive group.②HBV preS1 antigen was good coincide with HBV DNA that is a index of virus copies. ③The detection rate of HBV preS1 antigen was high in liver fuction （ALT 40 U/L）of chronic active hepatitis group. Conclusion The detection results of HBV preS1 antigen and HBV DNA may very well reflect the copies of HBV and the reactiveness condition of chronic active hepatitis. HBV preS1 antigen is acted as complement of HBeAg and HBV DNA,and a index that judges reactiveness of chronic hepatitis patients. HBV preS1 antigen has very high value for the diagnosis ,cure and observation of HBV.
