摘要
目的观察逆行插入交锁髓内钉治疗肱骨干骨折的疗效。方法采用肱骨交锁髓内钉逆行进钉内固定治疗肱骨干骨折14例,分别从手术情况、并发症、生物力学、骨折愈合情况、功能恢复等方面进行观察。结果本组所有14例患者均得到随访,随访6~20个月,平均8.4个月。骨折全部愈合,平均骨折愈合时间4.7个月。除开展该手术早期有1例患者进针部位出现裂纹骨折外,无感染、医源性血管神经损伤、锁钉断裂、再骨折等并发症,肩肘关节均恢复正常功能。其中肩关节功能根据Neer评定标准,得分91~100分,平均95.5分。结论肱骨干骨折逆行交锁髓内钉内固定术是治疗肱骨干骨折的安全有效内固定方法,具有骨折愈合快,术后并发症少等优点。
Objeetive To observe the treatment of humeral shaft fractures with retrograde interlocking intramedullary nailing. Methods Fourteen patients with humeral shaft fractures were treated by retrograde nail insertion with humeral intramedullary interlocking nail. Discoveries in operation, complications,biomechanics and bone healing were observed. Results All the 12 patients were followed up for an average of 8.4 months （6-20 months）. Bone union occurred in 14 cases. Delayed union occurred in 1 case. No nonunion and delayed union was found. There was 1 case of fissure fracture in the early stage of developing the operation. The average Neer＇s score of shoulder was 95.5. Conclusion The treatment of humeral shaft fracture by using retrograde intramedullary interlocking nail is reliable in fixation with fewer complications and faster healing.
