十二指肠营养管在食管贲门癌术后的临床应用

A Clinical Trial on Nutrition Support through Duodenum Tube after Operation of Esophagus and Cardia Carcinoma

目的分析食管癌贲门癌患者术中放置十二指肠营养管进行早期肠内营养支持,对维持机体营养,促进恢复及预防并发症的作用。方法768例食管癌贲门癌患者随机分成两组,肠内营养组(EN组)与肠外营养组(PN组)。EN组术中放置十二指肠营养管,术后第1 d即予以肠内营养。PN组术后第1 d予以周围静脉营养支持。两组患者监测其术前1 d、术后5 d、术后8 d的体重、血浆白蛋白、以及术后肠道功能恢复时间和出现的并发症。结果两组患者在体重、血浆白蛋白无明显差异。在术后肠功能恢复时间、术后并发症方面,EN组明显优于PN组(P<0.01)。结论食管癌贲门癌术中放置十二指肠营养管予以肠内营养,能很快纠正机体的负氮平衡状态,明显改善患者营养,并具有方法简单、安全、并发症少且费用低廉的优点。

Objective To evaluate the effect of duodenum nutrition tube on nutrition support after Operation of Esophagus and Cardia Carcinoma. Methods Patients were randomly divided into 2 groups： enteral group（EN） and parenteral group（PN）. The cases in EN group accepted duodenum nutrition tube during operation and nutrition liquid on 1st day. The patients in PN group accepted nutrition liquid through vein on 1st day. All cases＇ body weight, the level of plasma albumin and the time of gastrointestinal function recovery were observed on 1 st day before operation ,on 5th day and 8th day after surgery. Results there are no differences between two groups in body weight and the level of plasma albumin. The time of gastrointestinal function recovery in EN group was shorter than that in PN group （P 〈0.01）. Conclusion Nutrition support through duodenum nutrition tube is effective for patients with esophagus and cardio carcinoma.
