米索前列醇直肠给药预防剖宫产后出血78例临床观察

78 Cases Clinical Observation of Prevention of Post Cesarean Section Bleeding with Misoprostol by Anus after Operation

目的观察米索前列醇直肠给药用于预防剖宫产术后出血的效果。方法将2003年1月～2005年7月，在我院产科住院的剖宫产患者150例，随机分成两组，实验组78例，于剖宫产术后立即直肠内置入米索前列醇400μg。对照组72例，术后应用5％葡萄糖250ml，加入缩宫素20u静脉点滴，分别观察术后2h、24h阴道出血、术后2h宫底距脐部间距离及术后肠功能恢复时间等。结果实验组2h阴道出血（126±64）ml，24h阴道出血（160±42）ml，与对照组比较明显减少（P〈0．05）。结论米索前列醇直肠给药促进子宫收缩作用明显，能很好地预防剖宫产产后出血，且用药方便、安全。

Objective Observe the effect of misoprostol to prevent post cesarean section bleeding. Methods From January 2003 to July 2005 ,150 cases of cesarean section receptor with postpartum hemorrhage were divided into two groups randomly, the misoprostol experiment group is 78 cases who were given 400 μg misoprostol by anus after operation. The control group is 72 cases who were put on a drip with 250ml glucose 5% mixed with oxytocin for injection 20U after operation. Observe the bleeding in 2h ,24h , the contractions of the uterus in 2h and the.restore time of intestines function after operation . Results The bleeding quantity of the misoprostol experiment group is （126 ± 64）ml in 2h and （160 ± 42）ml in 24h. It is less than that of the control group clearly（P〈0. 05） .The space between the end of the uterus and the navel is （3.5 ±1.3）cm. It is larger than that of the control group（P 〈0.05）. The restore time of intestines function the misoprostol experiment group is （24.5 ± 3.5）h and the time is shorter than that of the control group clearly（P〈0. 05）. Conclusion It is clear that misoprostol given by anus after operation speeds the contractions of the uterus and not only does it prevent post cesarean section bleeding very well but also convenient and safe.
