椎弓根螺钉固定、椎体间融合术常见的技术错误及应对措施 被引量:3

摘要 目的提高应用椎弓根螺钉固定、椎体间融合术治疗腰椎退行性疾病的技术水平。方法随访分析我院2001年1月~2005年12月在我院应用该技术治疗腰椎退变性疾病的患者,复习其X线片,并与其术后X线对比,分析发生的技术失误及原因。结果获得随访的163例患者中,出现椎弓根螺钉置入方向错误者12例17枚螺钉,椎弓根螺钉过长者4例6钉,椎弓根螺钉连接棒脱落者2例2根棒,融合器移位者3例,Cage下沉者3例。分析失误原因,提出简单易行的椎弓根螺钉方位判断方法、该术式的正确的操作技术以及技术失误的预防措施。结论详细了解、全面掌握椎弓根螺钉固定、椎体间融合术的技术特点,才能够取得满意的临床疗效。 Objective To improve the surgical technique of PLIF in the treatment of lumber degenerative diseases. Methods From Jan 2001 to Dec 2005, the patients with degenerative lumber disease who underwent PLIF were reviewed. The clinical and radiological results were compared and evaluated. The cause of technical error was analyzed. Results A total of 163 patients were followed up, and the results showed that postoperative problems include 17 pedicle screw misdirection in 12 patients, 6 screw over-length in 4 patietents,2 rods unlock in 2 patients, 3 cages dislocation in 3 patients and 3 cages subsidence in 3 patients.After analysis the reason of failure,we proposed a simple method to make sure the accurate direction of the pedicle screw.The accurate technique of PLIF and the protective measures of technique error were also discussed.Conclusion The satisfactory clinical result is relied on the completely understanding the characteristics of pedicle screw fixation and intervetebral fusion.
