血管性认知损害与S100β和髓鞘碱性蛋白的关系 被引量:8

摘要 目的探讨S100β和髓鞘碱性蛋白(MBP)与血管性认知损害(VCI)的关系。方法①分组:将67例VCI患者分为轻度VCI组,25例;中度VCI组,23例;重度VCI组,19例;对照组为正常健康者30名。②用酶联免疫吸附法(ELISA)检测各组对象血清的S100β蛋白和MBP含量。③认知功能评价:运用简易智能状态量表(MMSE)及临床记忆量表(CMS)评估认知功能。结果轻、中、重度VCI组的血清S100β浓度分别为(0.42±0.08)、(0.59±0.06)及(0.64±0.06)μg/L,对照组为(0.39±0.11)μg/L,差异均有统计学意义(P<0.05);相关性分析显示,中、重度VCI组血清S100β含量与MMSE总分及记忆商值皆呈显著性负相关(r=-0.55,r=-0.57;r=-0.63,r=-0.65,),而轻度VCI组血清S100β含量与MMSE总分及记忆商值无显著相关性。轻、中、重度VCI组的血清MBP浓度为(3.94±0.68)、(3.44±0.76)及(5.21±0.96)μg/L,对照组为(1.75±0.47)μg/L,差异有统计学意义(P<0.05)。相关性分析显示,重度VCI组血清MBP含量与MMSE总分及记忆商值均呈显著性负相关(r=-0.67,r=-0.61),轻、中度VCI组血清MBP含量与MMSE评分和记忆商值均呈显著性负相关(r=-0.62,r=-0.58;r=-0.63,r=-0.57)。结论血清S100β和MBP浓度有可能作为观察中、重度血管性认知(包括记忆和脱髓鞘变化)损害的生化指标。 Objective To explore the relationship between S100β, myelin basic protein (MBP) , and vascular cognitive impairment (VCI). Methods ( 1 ) Grouping: 67 patients with VCI were divided into mild ( n = 25 ), moderate ( n = 23 ), and severe ( n = 19) groups; Normal healthy subjects (n =30) were used as control group; (2) Serum S100β and MBP levels in all groups were detected by enzyme linked immunosorbent assay (ELISA) ; and (3) cognitive function were evaluated by mini-mental state examination (MMSE) and clinical memory scale (CMS). Results The serum S100β levels in the mild, moderate and severe VCI groups were 0.42 ±0.08, 0.59 ±0.06 and 0.64 ±0.06 μg/L, respectively; It was 0. 39 ±0. 11 μg/L in the normal control group, and there were significant differences (P 〈 0. 05). The correlation analysis revealed that the serum S100β levels were both negatively correlated with the total MMSE scores and memory quotient (r = -0.55, r= -0.57; r= -0.63, r= -0.65) in the moderate and severe VCI groups, while the serum S100β levels were not significantly correlated with the total MMSE scores and memory quotient in the mild VCI group. The serum MBP levels in the severe, moderate, mild, cerebral infarction groups whom without cognitive impairment, and the normal control group were 5.21 ±0. 96, 3.44 ±0. 76, 3.94 ±0. 68 and 1.75 ±0. 47 μg/L, respectively, and there were significant differences ( P 〈 0. 05 ). The correlative analysis revealed that the serum MBP levels were both significantly negatively correlated with the total MMSE scores and memory quotient in the severe VCI group ( r = -0. 67, r = -0. 61 ); the serum MBP levels were also negatively correlated with the MMSE scores and memory quotient in the mild and moderate VCI groups (r = -0.62, r = -0.58; r = -0.63, r = -0. 57). Conclusion S100β and MBP levels may be used as the biochemical indicators for observing moderate to severe vascular cognitive impairment.
