摘要
目的探讨早期肠内营养(EEN)联合生大黄对实验性急性坏死性胰腺炎(ANP)大鼠肝损伤的影响。方法48只雄性Wistar大鼠随机分为ANP组、生大黄组、EEN组及EEN联合生大黄组(联合组),每组12只。各组于造模后48h测定血肿瘤坏死因子(TNF—α)、内毒素(LPS)、谷丙转氨酶(ALT)、谷草转氨酶(AST)、C反应蛋白(CRP)、丙二醛(MDA)含量;检测肝脏组织髓过氧化物酶(MPO)活性。结果与ANP组相比,其他3组的各项指标均有显著差异;联合组血TNF—α[(1.38±0.17)ng/ml]、内毒素[(0.114±0.048)EU/ml]、ALT[(90.95±55.67)u/L]、AST[(203.12±36.92)U/L]、CRP[(41.2±22.6)mg/L]、MDA[(4.78±1.55)nmol/ml]浓度及肝脏组织MPO活性[(3.95±0.71)u/g]均较生大黄素组与EEN组显著降低(P〈0.05)。结论EEN联合生大黄可有效减轻ANP并发的肝损伤程度,其机制可能与保护肠黏膜屏障功能、改善肝脏微循环障碍、抑制肝脏急性炎症反应、氧化反应和肝细胞凋亡等作用有关。
Objective To investigate the effect of early enteral nutrition(EEN) plus rhubarb on hepatic injury complicated by acute necrotizing pancreatitis (ANP) in rats. Methods 48 male Wistar rats were equally randomized into four groups: ANP group, rhubarb group (Rh group), early enteral nutrition group (EEN group) and EEN + rhubarb group (Corn group). 48 h after establishing the model, serum levels of tumor necrosis factor alpha (TNF-α), endotoxin, glutamic pyruvic transaminase (ALT), glutamic oxaloacetic transaminase (AST), C-reactive protein(CRP) and maleic dialdehyde (MDA) and myeloperoxidase (MPO) activity in liver tissue were detected in each group. Results Every index was significant difference in Rh group, EEN group and Corn group, compared with ANP group(P 〈 0.05). Serum levels of TNF-α(1.38 ± 0.17)ng/ml, endotoxin (0. 114 ± 0. 048) EU/ml, ALT(90. 95 ± 55. 67)U/L, AST(203. 12 ± 36. 92)U/L, CRP(41. 2 ± 22. 6)mg/L, MDA (4. 78 ±1. 55)nmol/ml and MPO(3. 95 ± 0.7l)U/g of liver tissue in Corn group were significomtry lower than that in Rh group and EEN group (P 〈 0. 05). Conclusions Administration of EEN + rhubarb reduced the severity of hepatic injury complicated by SAP, presumably by protecting the intestinal mucosa harrier, improving hepatic microcirculatory disturbance, inhibiting the acute inflammatory reaction, and oxidizing reaction of liver and hepatic apoptosis.
