痛风性关节炎的影像学诊断

Imaging diagnosis of gouty arthritis

目的研究痛风性关节炎的影像学特征。方法收集48例痛风性关节炎的影像学和临床资料并进行回顾性分析。其中男44例,女4例,平均年龄54±2.2岁,中位年龄56.5岁,平均病程(6±1.2)a。结果有40例于第一跖趾关节发病,表现为关节旁痛风结节肿块、外翘性穿凿样骨缺损、囊状骨破坏及痛风石钙化等影像学特征。结论本病反复发作后可显示出典型的影像学征象,诊断时应充分利用各种检查方法并密切结合临床资料。

Objective： To study the imaging characteristics of gouty arthritis. Methods： The imaging and clinical data of 48 eases of gouty arthritis were collected and analyzed retrospectively. There were 44 male and 4 female patients in this study. The mean age of 54±2. 2, median age of 56. 5, the mean disease duration at initial diagnosis was 3d～27（6 ± 1.2）a Results： The first metatarsophalangeal joint of the patients was involved at the earliest onset in 40 eases. Radiographies or CT demonstrated paraarticular eccentric gouty nodular masses, the overhanging margin of ＂ptmched out＂ erosions, cystic destruction and calcification of a tophus on X - ray film or CT scanning. Conclusion： Definitive diagnosis can be made with clinically regression and remitting of the disease. Typical radiographical finding combined with laboratory result, CT and MRI should be recommended in order to detect subtle lesions.
