摘要 目的研究乳腺癌手术前细针穿刺涂片免疫细胞化学检测(ICA)P53基因蛋白的表达及意义。方法对86例乳腺癌手术前进行FNA涂片,应用ICA乳腺癌P53基因蛋白在FNA涂片上的表达情况,并与术后病理组织切片上的P53基因蛋白的表达进行对照。结果乳腺癌FNA涂片ICA检测P53基因蛋白的阳性率为30.2%(26/86),相应病理组织切片的阳性表达率为27.9%(24/86),手术前ICA和手术后免疫组化检测(IHA)P53基因蛋白的表达结果具有显著的一致性(2χ=53.71,P<0.005),两种方法之间无差异性。P53基因蛋白在FNA涂片上的阳性表达率与乳腺癌的病理组织学分级(2χ=8.671,P<0.025)、临床分期(2χ=10.20,P<0.01)明显正相关,与雌孕激素受体显著负相关(ER:2χ=18.38,P<0.005.PR:2χ=6.351,P<0.025)。结论乳腺癌P53基因蛋白在FNA涂片上的表达和组织切片上的表达具有显著的一致性,无差异性。P53阳性是乳腺癌不良生物学行为的指标。本研究对乳腺癌的早期诊断、早期判断其生物学行为、指导治疗判断预后具有重要意义。 Objective: To study the expression and significance of P53 gene on the preoperative FNA smears of the breast cancer by immunocytochemical assaying(ICA). Methods Using immunocytochemical assay, we investigated the expression of P53 gene protein on FNA smears of 86 cases of preoperative breast cancer, and compared with its expression on the postoperative histological sections. Results The positive expression rate of P53 gene protein on the FNA smears of breast cancer by ICA was 30. 2% (26/86) , and it was 27.9% (24/86) on the postoperative histological sections. The results of P53 expression by ICA and immunohistochemical assay ( IHA ) had significant identity (x^2 = 53.71, P 〈 0. 005 ), and had no difference. There were positive correlation between expression of P53 on FNA smears and histological grading (x^2 = 8.761, P 〈0. 025 ) , between its expression and clinical TNM stages(x^2 = 10. 20,P 〈0. 01 ). But there was negative correlation between its expression and ER(x^2 = 18. 38, P 〈 0. 005 ) ,PR(x^2 = 6. 351,P 〈 0. 025 ). Conclusion: There were significant identity and no difference of P53 expression between on FNA smears and on the postoperative histological sections. P53 was a director of poor biological behavior of breast cancer. This study had important significance for diagnosing breast cancer early, judging their biological behavior early, directing their treatment and predicting their prognoses.
