脉冲振荡法测定慢性阻塞性肺疾病呼吸阻抗与常规肺功能检查指标的相关性分析 被引量:6

摘要 目的探讨呼吸阻抗及肺通气功能检查在慢性阻塞性肺病诊断中的相关性及互补性。方法对20例肺功能正常人和101例慢性阻塞性肺病患者脉冲振荡肺功能测定指标及常规肺功能检查资料进行比较。结果慢性阻塞性肺疾病患者与正常人相比,呼吸总阻抗(Z5)、总气道阻力(R5)、周边气道阻力(R5-R20)、共振频率(RF)、周边阻力(RP)均显著增高且与肺通气功能呈负相关;电抗(X5)明显降低,与肺通气功能呈正相关;其中,共振频率(RF)的相关性最为密切,共振频率(RF)、周边阻力(RP)随慢性阻塞性肺疾病严重程度增加而增加。结论脉冲振荡肺功能测定与常规肺通气功能检查在慢性阻塞性肺疾病的诊断中具有良好的相关性及互补性;共振频率和周边阻力可以作为判断慢性阻塞性肺疾病严重程度的指标。 Objective To assess the correlations between ventilation function and respiratory impedance in patients with COPD. Methods To compare the parameters of pulmonary ventilation function with the results of respiratory impendance measured by impulse oscillometry in 20 healthy adults and 101 patients with COPD. Results There was a significant increase in respiratory impedance ( Z5 ), respiratory resistance at 5Hz ( R5 ), R5 - R20, resonant frequency ( Rf), and peripheral resistance(Rp) in the chronic obstructive pulmonary disease group compared to those in the healthy group. Reactance at 5Hz(X5 ) in the chronic obstructive pulmonary disease group was significanty lower than that in the healthy group. ZS, R5, (R5 - R20), Rf and Rp were negatively correlated with parameters of pulmonary ventilation, while X5 was positively correlated. Of all indexes, the closest correlation was found between R.f with forced expiratory volume in one second ( FEV1 ) and the severity of the chronic obstructive pulmonary disease. Rf and Rp were positively correlated with disease severity. Conclusion There are good correlations and complementarities between pulmonary ventilation function with respiratory impendance measured by impulse oscillometry in the diagnosis of chronic obstructive pulmonary disease. Rf and Rp can used as indexes for evaluating the severity of chronic obstructive pulmonary disease.
