放疗同时射频热疗治疗晚期宫颈癌的临床观察

Evaluation of Short-term Effect of Radiotherapy Combined With Radiofrequency Hyperthermia in Advanced Cervical Caner

目的:观察放疗热疗同步治疗宫颈癌的近期疗效及急性毒副反应。方法:初次治疗的Ⅱb-Ⅲb期宫颈癌患者40例。放疗采用加速器体外照射+192Ir腔内后装治疗,体外剂量50Gy/25f/5W(30Gy时中央挡铅),腔内照射A点总剂量42Gy/6f。射频热疗方案:热疗机频率40.68MHz,入射功率700-950W,反射功率7-30W,时间60-70min/次,每周2次,共6-10次。结果:CR为70%,PR为23%,总有效率93%。治疗期间的不良反应为轻度的骨髓抑制和胃肠道反应,以及皮下脂肪硬结3例(7.5%)。结论:放疗热疗联合应用,能提高晚期宫颈癌局部控制率,同时可减轻放疗的急性反应,患者耐受性好。

Objective： To evaluate the short- term effect and acute toxicity of SR - 1000 radiofrequency （RF） combined with radiotherapy in advanced cervical cancer. Methods： From september 2003 to December 2006,40 stage Ⅱ b- Ⅲ b cervical cancer patients confirmed pathologically were treated with Radiotherapy was ad ministered with one - daily external irradiation do ses of 2Gy to the pelvis, 5d/wk and brachytherapy to deliver a total dose 42Gy to point A. The temperature of pelvic cavity was in Creased and maintained at 40.5-42.5℃ for 60-90 minutes with SR- 1000 RF, Hyperthermia was given twice per week, with the whole treatment lasting for 3-5 weeks. Results： complete remisson （CR） was observed in 28 cases and PR in 9 cases, the total response rate was 93 %. The common sideeffects were fat necrosis （7.5 % ） on RF, and acute toxlaity reaction increased but was to lerable. Conclusion： SR- 1000 radiofrequency combined with radiotherapy may improve the short - term therapeutic efficacy of advanced cervical cancer and the patients can endure the side effects.
