胆囊癌p53蛋白表达(附67例分析)

p53 protein expression in carcinomas of gallbladder

目的：检测６７例胆囊癌、１０例胆囊腺瘤及癌旁粘膜的ｐ５３蛋白表达，分析其与肿瘤组织学类型的关系。方法：应用免疫组化技术。结果：ｐ５３蛋白胆囊癌阳性表达为５９．７％，腺瘤为２０％。阳性细胞主要分布在腺瘤的异型增生区；癌旁粘膜ｐ５３蛋白阳性表达在异型增生上皮２／４例，单纯增生上皮５／１４例，肠化上皮１／２例。腺癌ｐ５３蛋白阳性率Ⅰ级４８．４８％，Ⅱ级９０．９％，Ⅲ级７１．４２％，差异有显著性（Ｐ＜０．０５）；Ⅰ级腺癌中，乳头状腺癌阳性率高达６６．６６％。结论：腺瘤及癌旁粘膜中的异型增生、化生及单纯性增生上皮ｐ５３蛋白过度表达，均有发生恶变的潜在可能。

Objective:To study the expression of p53 in 67 gallbladder carcinomas,10 adenomas and adjacent mucosa.Methods:Immunohistochemical technic was used for detection.Results:59.7% of the carcinomas and 20% of the adenomas were positive in staining and the positive cells focus mainly on dysplastic areas of adenoma.In all cases of the adjacent mucosa examined,p53 protein was positive in 2/4 with metaplasia.In the histological studies,p53 positive in well,intermediate and poorly defferentiated adenocarcinoma were 48.48%,90.90%and 71.42% respectively.p53 protein were positive in 66.66% of papillary subtypes,higher than other subtypes in the well defferentiated carcinoma.Conclusion:It suggests that,over expression of p53 protein in dysplasia,metaplasia and hyperplasia of adenoma end mucous membrane of gallbladder carcinoma is potential for malignancy.
