膀胱粘膜隧道法尿道成形术治疗尿道下裂22例

Urethroplasty with tunneling of bladder mucosa in treatment of hypospadias

目的：尿道下裂手术方法很多，然而较高的手术后并发症对泌尿外科医生仍是一个挑战。本文研究目的旨在减少和消除手术并发症，提高尿道下裂手术成功率。方法：采用膀胱粘膜隧道法对２２例Ⅱ期尿道下裂行尿道成形术，同时转移阴囊皮瓣覆盖阴茎根部切口，以对防止尿瘘和缓解阴茎根部切口皮肤缝合过紧的缺点。结果：全部２２例外观和功能均获得满意，无尿瘘及尿道狭窄严重并发症。本术式特别适用于尿道口位于或接近阴茎根部的Ⅱ期尿道成形术。结论：本组资料显示，该术式对减少和消灭尿道下裂术后并发症有良好的效果，可在临床上广泛推广和应用。

Objective:There are many operative methods for correction of hypospadias.However,the high postoperative complications remain a challenge to the urologists.The purpose of this investigation is to reduce or eliminate the complications and to improve the result of operation.Method:Urethroplasty is made in 22 cases of hypospadias in 2 stages by tunneling of the bladder mucosa and covering the incision with scrotal skin flap to lessen the tense of the skin at the foot of penis.Results:The results of the operation were satisfactory in appearance and function.No serious complications,such as urinary fistula and urethrostenosis occured.The operation is suitable for the cares with urinary meatus located at or closed to the root of penis.Conclusion:This operation described above is effective in reducing and eliminating the postoperative complications of the hypospadias.It can be spreaded to be used in the clinicl practice.
