自制扩张套管扩孔后空心钉固定治疗老年股骨颈骨折

Hollow Nail Internal Fixation Reamed by Self-made Expanding Bolsters for Senium Femoral Neck Fractures

目的探讨自制扩张套管扩孔后空心钉内固定治疗老年股骨颈骨折的手术方法及疗效。方法老年股骨颈骨折52例,根据Garden分型:Ⅰ型1例,Ⅱ型5例,Ⅲ型32例,Ⅳ型14例,均予自制扩张套管扩孔后空心钉内固定治疗。结果52例中48例获得1～4年随访,4例失访。2例因为内科疾病死亡;骨折愈合46例(95.8%),其中发生股骨头坏死1例,予对症治疗后生活能自理。结论自制扩张套管扩孔后空心钉内固定治疗老年股骨颈骨折,手术操作方便、固定牢靠、损伤小、痛苦少,是一种较为有效的方法。

Objective To explore the operative processes and clinical efficacy of hollow nail internal fixation reamed by serf-made expanding bolsters for senium femoral neck fractures. Method 52 aged patients of femoral neck fractures received the operations mentioned above,with 1 of Type Ⅰ , 5 of Type Ⅱ , 32 of Type Ⅲ, and 14 of Type Ⅳ according to Garden Typing. Result 48 of 52 got 1 to 4 years follow-up. 2 cases died of non-orthopaedics diseases, and 46 cases （95.8%） got synostosis, in whom 1 suffered femoral head necrosis. Conclusion It is recommended as easy operation, stable fixation, mini-trauma of the hollow nail internal fixation reamed by serf-made expanding bolsters for senium femoral neck fractures.
