外伤性白内障后囊破裂的一期后房型人工晶体植入术

Primary Posterior Chamber Intraocular Lens Implantation in Traumatic Cataract With Posterior Capsule Breaks

本文对３２例（３２眼）因眼球穿通伤而致晶体后囊膜破裂的病例，根据其后囊膜破裂的位置和大小，采用不固定、单袢固定和双襻固定三种方法施行后房型人工晶体植入术。术后视力在０．５或以上者２８例（８７．５％），０．１—０．４者４例（１２．５％）。并发症包括术中睫状体出血２眼（６．２５％），后囊膜裂口增大２眼（６．２５％）；术后前房出血６眼（１８．８％），暂时性眼压偏高６眼（１８．８％）和暂时性眼压偏低７眼（２１．８％）。结果表明，在晶体后囊膜裂口小的情况下植入后房型人工晶体，可以不固定晶体襻或仅固定其一襻。

Thirty two consecutive patients of traumatic cataract with posterior capsule braeks caused by penetrating eye trauma were retrospected.Posterior chamber intraocular lenses were implanted in all these patients with three techniques,ie,without fixation,with single haptic fixation and with both haptics fixation,according to the position and size of the posteior capsule.Postoperative visual acuity was 0 5 or better (corrected) in 28 cases (87 5%) and 0 1 - 0 4 in 4 patients (12 5%).Intraoperative problems included ciliary body bleeding in 2 patients(6 25%)and enlargement of posterior capsule breaks in 2 patients (6 25%).Postoperative complications included hyphema in 6 eyes (18 8%),transient intraocular pressure elevation in 6 eyes(18 8%),transient hypotony in 7 eyes(21 8%).Not all PC IOLs have to be fixed by two haptics.In patients with small posterior capsule breaks,PC IOL may not be fixed or fixed by only one haptics.
