难治性癫痫相关耐药基因的研究 被引量:1

摘要 癫痫是一组由大脑神经元异常放电所引起的以短暂中枢神经系统功能失常为特征的慢性脑部疾病,是危害人类健康的疾病之一。如何有效控制癫痫发作,提高患者生活质量,已经越来越成为医学界所关注的问题。长期正规抗癫痫药物(anti-epileptic drugs,AEDs)治疗被认为是控制发作的最佳治疗方案。尽管目前已有20余种药物经临床证实对控制发作有效,尚有约30%的患者在经多种AEDs正规治疗并已达最大治疗剂量后,仍然不能有效控制发作,发展成为难治性癫痫。值得注意的是,在难治性癫痫的患者中,多数表现为对多种AEDs的耐受,尽管这些药物具有不同的结构和作用机制。这就使我们认识到,可能存在一种共同的机制影响着AEDs的作用。目前,在这方面研究中取得一定进展,主要为多药转运体过度表达机制,即由于多药转运体的过度表达,使AEDs在通过血脑屏障时被主动泵出的量增加,使局部的AEDs达不到有效的治疗浓度,从而导致癫痫的难治性。近些年对肿瘤耐药机制的研究,使该理论得到了广泛的支持与关注,而对于与之相关的多药耐药(multiple drugs resistance,MDR)基因及其表达产物的研究,也逐步成为目前研究的热点。
