摘要
目的探讨肺动脉血栓栓塞所致兔急性肺动脉高压模型的左心功能变化及其意义。方法模拟人体血栓栓子脱落造成急性肺动脉栓塞的过程建立兔急性肺动脉高压模型,同步采用右心导管测定肺动脉收缩压及超声评价栓塞前、后左心功能。结果与栓塞前相比,栓塞后心腔几何形态显著改变,每搏量明显减低(P<0.05),左室射血分值、小轴缩短率变化无统计学差异(P>0.05),二尖瓣E峰峰值血流速度减低,E/A<1,组织多普勒e/a<1(P<0.05)。结论兔急性肺动脉血栓栓塞性肺动脉高压可以导致左心舒张功能减低,而左心收缩功能影响较小。
Objective To explore the left ventricular function of a rabbit model with acute thrombo-embolic pulmonary hypertension (APH) monitored by echocardiography. Methods Imitated the patho-physiological process of pulmonary thrombo-embolism, a rabbit model with APH was established after its self-embolism' inpouring, whose pulmonary artery pressure was measured simultaneously by right heart catheter guided by echocardiography. Then determined the left ventricular function pro-embolism and post-embolism by echocardiography. Results The appearance of chambers heart changed prominently during post-embolism, as compared with pro-embolism. Stroke volume of left ventricle degraded obviously (P〈0.05). Ejection fraction and shortening fraction had no significance (P〉0. 05). Peak value of wave E diminished. E/A〈1 and e/a〈 1. Diastole function of left ventricle decreased obviously (P〈0.05). Conclusion The increase in acute pulmonary hypertension itself caused a left ventricular diastolic severe impairment.
