停跳与不停跳冠状动脉搭桥术围术期应激反应因子的比较 被引量:5

摘要 目的 比较停跳与不停跳冠脉搭桥术围术期应激反应因子,探讨不停跳冠状动脉搭桥术后早期疗效优势的原因。方法 2004年10月-2005年4月我院30例冠状动脉硬化性心脏分为2组,15例行停跳冠脉搭桥术(ONCAB组),15例行不停跳冠脉搭桥术(OPCAB组),分别在围术期7个时点抽血检测热休克蛋白70(heat shock protein 70,HSP70)、TNF-α、cTnT、CK-MB。结果 2组病人术后血清HSP70均明显上升,术后2h达最高峰,与术前相比差异有显著性(P〈0.01);ONCAB组术后血清HSP70升幅明显高于OPCAB组(P〈0.01)。ONCAB组术后cTnT、CK-MB、TNF-α浓度升幅较OPCAB组大,差异有显著性(cTnT,P〈0.01;CK-MB,P〈0.05;TNF-α,P〈0.01)。HSP70峰值浓度与cTnT、CK-MB、TNF-α峰值浓度呈显著正相关(r=0.632,P=0.025;r=0.673,P=0.033;r=0.745,P=0.013)。ONCAB组体循环中HSP70峰值浓度与主动脉阻断时间有显著相关性(r=0.782,P=0.004)。结论 ONCAB组病人组织细胞损伤、机体应激反应均较OPCAB组严重。
