FHIT mRNA表达与结直肠癌新辅助化疗疗效的相关性

Correlation between different expression of FHIT mRNA and therapeutic effect of neoadjuvant chemotherapy for colorectal cancer

目的 探讨脆性组氨酸三联体（FHIT）mRNA表达与结直肠癌新辅助化疗疗效的相关性。方法 检测51例局部进展期结直肠癌患者术前FHIT mRNA表达和细胞凋亡指数（AI），观察2个周期FOLFOX-4方案新辅助化疗后FHIT mRNA和AI的变化。结果 化疗后结直肠癌组织中AI为9．58±3．21。显著高于化疗前6．24±2．38，P〈0．01；化疗前FHIT mRNA高表达患者化疗前后平均AI差值为4．57±1．13，显著高于FHIT mRNA低表达患者的2．74±0．45，P〈0．01；化疗前后结直肠癌组织中FHIT mRNA表达差异无统计学意义。结论新辅助化疗可明显促进结直肠癌细胞的凋亡，FHIT mRNA的表达水平与新辅助化疗疗效具有显著的相关性，FHIT mRNA的表达水平有可能成为进展期结直肠癌化疗疗效的预测指标。

Objective To investigate the correlation between different expression of fragile histidine triad（FHIT） mRNA and therapeutic effect of neoadjuvant chemotherapy （NCT） for colorectal cancer. Methods Colonoscopy and biopsy were used to diagnosis in 51 cases of local progressive colorectal cancer. Oxaliplatin was administered on day 1 at the dose of 85 mg/m^2 as a 2 hours infusion, concurrently with calcium folinate 200 mg/m^2 ,day 1,2, followed by bolus 5 - FU 400 mg/m^2 and 22 hours infusion of 5 - FU 600 mg/m^2 for 2 consecutive days. The treatment was repeated every 2 weeks until the operations taken after 56 days. In situ hybridization and TUNEL were used to observe the variation of FHIT mRNA expression and apeptotic index （AI）. Results AI in colorectal cancer before and after NCT were 9.58 ± 3.21 and 6.24 ±2.38 respectively, there was significant statistical difference between before and after NCT （P〈0.01）. The difference of AI between before and after NCT of FHIT mRNA high expression in colorectal cancer was 4.57 ± 1.13, it was significantly higher than 2.74 ± 0.45 of FHIT mRNA low expression （ P 〈 0. 01 ）. There was no statistical difference of FHIT mRNA expression between before and after NCT （P 〉 0.05）. Conclusions NCT is effective for stepping up the apeptesis, FHIT mRNA expression levels are significantly associated with colorectal cancer chemesensitivity and it can be used as a predictive marker of response to chemotherapy in the patients with colorectal cancer.
