颅内浆细胞肉芽肿（14例报告及文献复习）

Intracranial plasma cell granuloma （ 14 cases report and review of the literature）

目的 探讨中枢神经系统浆细胞肉芽肿的病因、发病机制、诊断及治疗方法。方法 回顾性分析14例颅内浆细胞肉芽肿患者的临床资料，总结有效的诊断及治疗方法。左顶叶占42．9％（6例），右颞叶及右额叶各占14．3％（2例），单发病灶占92．9％（13例）。头颅CT检查显示多数为高密度影，占57．1％（8例）；MRI检查显示多数为T1WI显示等或稍低信号、T2WI显示高信号。结果 14例患者均经手术治疗，均痊愈出院。结论 颅内浆细胞肉芽肿缺乏特异性表现，易误诊，须依靠病理检查确诊，手术切除为主要治疗手段。

Objective To explore the etiology, mechanism, diagnosis and treatment of plasma cell granuloma in central nervous system. Methods Clinical manifestations of 14 cases of intracranial plasma cell granulomas were studied retrospectively. Three male and 11 female cases with age ranging from 23 to 71 years old were studied and 10 cases were found before the age of 46 years old. The lesions located at left parietal lobe in 6 patients（42.9% ）, 2 patients（ 14.3% ） with lesions located at fight temporal lobe, right frontal lobe respectively. Solitary lesion was found in 13 patients（92.9% ）. The tumors were hyperdense on CT scan in 8 cases（57.1% ）. The tumors were hypointense or isointense on T1WI of MRI, with increased signal on T2WI. Result All cases were operated and cured. Conclusions There is no specific manifestation for plasma cell granuloma and it is easier to be misdiagnosed. The final diagnosis should be confirmed by using pathologic approach and operation is the major method for curing.
