冠状动脉狭窄对应左室心肌纵向运动速度的定量研究 被引量:2

摘要 目的探讨定量组织速度成像(QTVI)技术与冠状动脉狭窄程度之间局部心肌运动速度变化的定量关系。方法应用定量组织速度成像技术对31例冠状动脉造影证实的冠状动脉狭窄患者(狭窄组)和21例正常人(对照组)左室心肌的12个节段测量QTVI曲线的收缩期峰值速度(Vs)、舒张早期峰值速度(Ve)和舒张晚期峰值速度(Va)。结果Vs、Ve在左室基底段和中间段与正常组比较均具有显著性差异(P<0.01或P<0.05),Va则无统计学意义。轻、中、重度狭窄组之间各指标相比均无显著性差异(P>0.05)。结论定量组织速度成像的Vs和Ve能较准确地检出冠心患者静息状态下的异常心肌节段,但对区分冠状动脉的狭窄程度似无显著作用。 Objective With quantitative tissue velocity imaging(QTVI),to observe the left ventricular myocardial longitudinal movement in coronary heart disease and to find out the relationship between it and the degree of stenosis in coronary artery. Methods Twenty-one normal subjects (control group) and 31 cases (stenotic group) with coronary heart disease verified by coronary arteriography were included. In each case,12 left ventricular segements were analyzed and velocity at systole (Vs),early diastole (Ve) and artrial contraction (Va) were obtained and compared between two groups. Results Compared with control group, Vs and Ve at base-and mid-segement were significantly lower in the stenotic group (P〈0. 01 or P〈0. 05). No obvious difference was found in each index among mild-,mid- and sever-stenotic group. Conclusions Using Vs and Ve measured with QTVI,we can accurately detect the abnormal segements in patients with coronary artery stenosis at rest ,but it seems no helpful to determine the degree of coronary artery stenosis.
