疝环充填式无张力疝修补术后并发症38例分析

Clinical Analysis of Complications after the Mesh Plug Tension-free Inguinal Hernioplasty

目的探讨疝环充填式无张力疝修补术后并发症发生的原因及其预防与治疗措施。方法对我院2000年-2005年间疝环充填式无张力疝修补术217例临床资料作回顾性分析。结果217例中发生各种并发症有38例,发生率17.5%,其中（术后持续）局部疼痛9例,伤（切）口皮下、阴囊积液6例,伤（切）口（皮下脂肪）液化2例,患侧睾丸肿大、萎缩3例,局部硬块异物感、局部隆起16例,伤（切）口感染,窦道形成2例。均治愈出院。结论疝环充填式无张力疝修补术是安全有效的术式,降低术后并发症的关键在于熟悉腹股沟的解剖,提高手术操作技巧,加强无菌原则及围手术期的治疗。

Objective： To investigate the cause of complications occurred after the Mesh Plug tension - free inguinal hemioplasty and the measures of prevention and treatment of these complications. Method： A retrospective analysis was made in 217 patients receiving Mesh Plug tension- free hemioplasty from 2000 to 2005 in our hospital. Results： 38 [ thirty- eight （17.5 %）] eases occurred various complications. Of them, persistent postoperative local pain happened in 9 eases, hydroeele of scrotum or ineisional hypodermia in 6 eases, liquefaction of subcutaneous tissue in 2 eases, swell or atrophy of ipslateral testis in 3 eases, local hard mass with foreign body sensation or local bulge in 16 eases, and sinus tract formed in 2 eases. All the eases were cured to leave hospital. Conclusion： The Mesh Plug tension - free inguinal hemioplasty is safe and efficient, the key to reduce the operation complications lies on the familiarity of the groin＇ s anatomy, skilled surgical technique, strieted asepsis principles and care ful perioperative management.
