摘要 目的探讨随意尿白蛋白/肌酐比值(ACR)的变异性及与24小时尿白蛋白(24hUAlb)的一致性。方法70例糖尿病患者(组1),分别留取随意尿和24小时尿各一次。67例糖尿病(组2),不同日留取3次随意尿。放射免疫法测定尿白蛋白,苦味酸法测定尿肌酐。结果(1)糖尿病1组ACR与24hUAlb相关性好(r=0.795,P=0.000),并且二者判断白蛋白尿一致性好(Kappa=0.8462)。(2)糖尿病2组的3次随意尿ACR的相关性分别为r1-2=0.884,P1-2=0.000;r1-3=0.929,P1-3=0.000;r2-3=0.879,P2-3=0.000,第一次与第二次、第一次与第三次、第二次与第三次和第一、二、三次判断白蛋白尿的一致率分别为92.54%、92.54%,88.06%、86.57%。结论随意尿ACR与24hUAlb相关性好,但存在变异。 Objective: To investigate the variability of random albumin- to - creatinine ratio (ACR) and its consistency with 24- houre urinary albumin (24hUAlb). Methods: 70 diabetic subjects (group 1) collected random urinary and 24 - hour urinary once, respectively. 67 diabetic subjects (group 2) collected random urinary in different three days. Urinary albumin was assayed by radioimmtmoassay and creatinine by picfic acid method. Results: ( 1 ) In group 1, ACR was nearer linearity with 24UAlb (r= 0.795, P = 0.000) and the consistency of ACR and 24hUAlb wasgood (Kappa= 0.8462). (2) The correlation of three morning ACR had nearer lineafity (r1-2 = 0.884, P1-2 = 0.000; r1-3 = 0.929, P1-3 = 0.000; r2-3 = 0.879, P2-3 = 0.000). Consistencies of ACR in group 2 were 92.54 %, 92.54 %, 88.06 %, 86.57 %, respectively. Conclusions: The correlation of random urinary ACR and 24hUAlb was good, however, variability existed, samples should be collected in differentd ays.
