摘要 射频消融是目前最先进和最有效的肝癌局部消融技术之一,在临床的应用也逐渐广泛,为小肝癌的治疗带来了新的希望和新的治疗模式。射频消融已被认为是小肝癌继外科切除术和肝脏移植术之后的根治性治疗方法。本文回顾了射频消融的原理及其发展;总结和评价了射频消融在小肝癌临床治疗中的疗效、安全性和综合应用情况,并与手术切除进行了对比;提出了射频消融存在的问题,展望了其进一步发展方向。笔者认为射频消融治疗小肝癌安全性高、并发症少、适应证广,疗效与手术切除相近,但评价其长期疗效还需要更长时间的随访观察和更多的前瞻性随机对照研究。 Radiofrequency ablation (RFA), one of the most advanced and effective Iocoregional ablative therapeutic methods, is widely used in treating hepatocellular carcinoma (HCC). It brings new hope and treatment pattern for small HCC, and has been regarded as a curative method as well as surgical resection and liver transplantation. In this article, we reviewed the principle and development of RFA, summarized and evaluated the therapeutic effect, safety and application of RFA in the treatment of small HCC, and compared it with surgical resection, raised current problems, and suggested the development direction. According to our experience over 6 years, the prognosis of small HCC after RFA is comparable to that after surgical resection, while RFA is safer, with less complications and wider indications, but its long-term efficacy needs to be observed in more prospective randomized clinical trails for a longer time.
