绝经后子宫内膜癌临床及病理特点分析

The study of the clinical and pathological characteristics of endometrial carcinoma of postmenopausal patients

目的:探讨绝经与子宫内膜癌的关系。方法:分析1997年1月～2002年12月128例绝经后子宫内膜癌患者资料,并以同期未绝经的105例子宫内膜癌患者作为对照,比较其临床病理特点,用寿命表法计算两者的生存率。结果:绝经后子宫内膜癌高血压的发生率显著高于未绝经组,而未绝经组中肥胖及不孕的发生率高于已绝经组。比较两组的高危病理类型、病理分级、肌层浸润深度、手术病理分期及5年存活率均有统计学差异。结论:绝经后发生的子宫内膜癌预后差与其临床病理特点有关。

Objective： To explore the relationship between endometrial carcinoma （EC） and menopause. Methods： A total of 233 patients with EC treated in this hospital from January 1997 to December 2002 were analyzed. 128 postmenopausal EC patients were compared with non - menopausal EC patients to evaluate the clinical and pathological characteristics of endometrial carcinoma. The 5 - year survival was calculated by life table method. Results： The incidence of hypertension increased, but that of inferility and obese decreased in postmenopausal group. The high risk pathological type, histologic grade, depth of myomerial invasion, operative pathology stage and 5 - year survival were significantly different between postmenopausal group and control group. Conclusion： The clinical and pathological characteristics of EC are responsible for the poorer prognosis of postmenopausal patients campared with those in nonmenopausal patients.
