宁夏回族不同人群膝骨性关节炎X片比较研究

Comparative study on X-ray of the knee osteoarthritis in Ningxia moslem

目的比较宁夏地区回族礼拜、不礼拜人群膝关节骨性关节炎(KOA)患者的病变程度在X线片上表现的差异,探讨回族KOA与礼拜的相关性。方法收集宁夏地区回族中老年人KOA患者290人456例膝,按是否礼拜分为礼拜组与非礼拜组。临床诊断采用美国风湿病学院推荐的临床标准,X线分级采用Kellgren和Lawrence提出的骨性关节炎X线分级修订标准。患者拍摄正位片及髌骨轴位片,测量股胫关节内、外侧间隙值、FTA角等指标,比较礼拜组与非礼拜组在X线片上的差异。结果礼拜组在膝关节间隙狭窄、内翻畸形的阳性率高于非礼拜组,X线分级程度大于非礼拜组,两组差异有统计学意义(P<0.01)。髌股不稳指标测量,礼拜组的阳性率高于非礼拜组,两组差异有统计学意义(P<0.01)。结论(1)宁夏地区回族礼拜人群KOA患者膝关节间隙狭窄、内翻畸形、髌骨不稳的程度都严重于非礼拜人群。(2)礼拜导致膝关节反复的应力负荷,加速KOA退行性变的程度。

Objective To compare the difference of X - ray between the churchgoer＇ s group and the not churchgoer＇ s group in Ningxia moslem which were diagnosed as knee osteoarthritis （KOA） and to explore the relationship whether they went to church. Methods 456 knee from 290 KOA patients of middle - old age in Ningxia were collected and were divided into two groups by weather going to church. The American College of Rheumatology was used as clinical diagnosis standard, Kellgren and Lawrenc＇ s standard was used as x - ray grade. The difference after the anteroposterior and axial patell x - ray of each one were taken and lateral joint space value, FFA angle and other angles were measured. Results The positive rate of churchgoer group in Lateral joint space value, FFA angle and other angles was higher than those of no churchgoer group, （ P 〈 0.01）, and X - ray grading was severer than the no churchgoer group （ P 〈 0.01 ）, there was difference between churchgoer group and no churchgoer group. Conclusion The churchgoer group is much severer on joint space value, varus or valgus deformity and unstable patella than those of no churchgoer group. Doing pray may lead to overload of knee and speed up degeneration of knee.
