自发性蛛网膜下腔出血治疗体会

Treatment of the spontaneous subarachnoid hemorrhage

目的总结自发性蛛网下腔出血的治疗经验。方法总结外科治疗92例自发性蛛网膜下腔出血患者的临床资料,包括造影示动脉瘤的部位,手术时机的选择,术中的处理技术。结果造影75例中阳性66例。55例手术患者恢复良好39例,一般14例,死亡2例;3例栓塞者恢复良好;8例未行手术者恢复良好3例,死亡5例;造影阴性9例中恢复良好9例;17例未行造影者,死亡5例。结论自发性蛛网膜下腔出血病例行脑血管造影,早期发现动脉瘤并行手术治疗能有效改善患者预后。

Objective To summarize the experience of treating patients with spontneous subarachnoid hemorrhage. Methods The data from 92 patients with the spontaneous subarachnoid hemorrhage surgically treated were studied retrospectively, including the site of aneurysms, the timing of surgery ,and the general neurosurgical techniques for operating on aneurysms. Results 66 aneurysms were found in the 75 patients undergoing the angiography. Among 55 cases undergoing the surgery,39 recovered well, 14 in general condition,and 2 died. 3 cases undergoing the embolization recovered well. For the 8 cases who didn＇t undergo the surgery,3 cases recovered well and 5 cases died. 9 cases who were not found aneurysm during the angiography recovered well. For the 17 cases who didn＇t undergo the angiography,5 cases ended in death. Conclusion To do the angiography for the spontaneous subarachnoid hemorrhage cases,early discovery and surgery is a good way to improve the prognosis of the patients.
