以草酸铂为主的联合化疗方案治疗晚期胃癌的临床观察

The Curative Effect of Combined Chemotherapy Mainly Using Oxaliplatin on Advanced Gastric Cancer

目的观察草酸铂（L-OHP）联合氟脲嘧啶（5-Fu）、甲酰四氢叶酸钙（CF）和羟基喜树碱（HCPT）治疗晚期胃癌41例的疗效。方法用L-OHP 85 mg/m^2加于葡萄糖注射液中静滴2 h,d1,15;CF 150 mg/m^2静脉滴注,d1,15（输注5-Fu前用药）;5-Fu 400 mg/m^2静脉推注,d1,15,5-Fu 2.4-3.0 g便携式微泵帮注入（Civ）48 h,d1,15,HCPT6 mg/m2静脉滴注,d1-5,每30 d为1周期,2周期评价疗效。结果本方案总有效率为56.1%,中位缓解期为8.5个月,中位生存期为10.4个月。并发症以周围神经毒性为主,恶心、呕吐反应轻,Ⅳ度骨髓抑制少。结论L-OHP联合CF、5-Fu和HCPT方案治疗晚期胃癌,疗效好、毒副反应轻、耐受性好,值得进一步临床推广应用。

Objective To observe the curative effect of combined chemotherapy using oxaliplatin, lencovorin, 5-fluorotiracil, and hyclroxycamptothecine, on advanced gastric cancer. Methods This mixture was put into dextrose injection liquor, 85 mg/m^2 L-OHP was mainlined for 2 hours,d1.15 ; 150 mg/m^2 CF was mainlined, d1.15 ; 400 mg/m^2 5-Fu was injected via vein ,d1.15 ; 2.4 -3.0 g 5-Fu was pumped by portable micro-pump for 48 hours, d1.15 ; 6 mg/m^2 HCPT was mainlined, d1-5^＊ One cycle was 30 days, and the curative effect was evaluated after 2 cycles. Results 56.1% patients had curative effects. The average period of relife was 8.5 months, and the average period of survival was 10.4 months. The therapy caused toxicity on peripheral nerve, mild retching, and little Ⅳ inhibition on bone marrow. Conclusion This combined chemotherapy was worthy of being extended in clinic, for it had good curative effect on advanced gastric cancer, though with some mild side effect.
