慢性胃炎胃粘膜组织幽门螺杆菌感染与π类谷胱甘肽S-转移酶的相关性研究 被引量:1

Study on the Relationship Between Helicobacter Pylori-infectionand and Glutathione S-Transferase-π in Gastric Mucosa with Chronic Gastritis

摘要 目的:探讨谷胱甘肽S-转移酶(GST-π)在慢性胃炎持续幽门螺杆菌(HPylori)感染胃粘膜组织中的变化及其与HPylori感染的关系。方法:将内镜及胃粘膜活检病理证实的138例慢性胃炎(其中慢性浅表性胃炎96例,慢性萎缩性胃炎42例),分别用13C呼气实验及病理切片改良Giemsa染色检测HPylori,按HPylori检测结果分为HPylori阳性组(87例)及HPylori阴性组(51例);用S-P免疫组化检测GST-π,观察与比较两组间胃粘膜组织中GST-π的表达,计算各组GST-π表达率,将所得结果输入spss统计学软件进行统计学处理。结果:本研究HPylori阳性87例,平均阳性率63.04%;GST-π阳性共58例,平均阳性率为42.01%。其中慢性浅表性胃炎组HPylori阳性61例,阳性率为63.54%;慢性萎缩性胃炎组HPylori阳性26例,阳性率为61.91%,两组间HPylori阳性率无显著差异(χ2=-0.0006,P>0.05)。慢性浅表性胃炎组GST-π阳性37例,阳性率为38.54%;萎缩性胃炎组GST-π阳性21例,阳性率50%,两组间阳性率无显著差异(χ2=1.14,P>0.25)。两组合并后HPylori阳性亚组中GST-π阳性率为32.18%显著低于HPylori阴性亚组的GST-π阳性率58.82%(χ2=8.30,P<0.005)。结论:持续HPylori感染可降低胃粘膜GST-π的表达,削弱GST-π对胃粘膜损伤的保护作用。 Objective: To observe the expression of glutathione S-transferase-π(GST-π) in gastric mucosa with chronic gastritis and Hp infection,and analyze virulence effect of Helicobacter pylori (H Pylori)in the development of gastric mucosal damage. Methods: 138 patients with chronic gastritis confirmed by endoscope and pathology were taken in this study(including 96 patients with chronic superficial gastritis and 42 cases with chronic atrophic gastritis). H Pylori infection was confirmed by 13 C breath test and histological examination on biopsy specimens with Giemsa dyeing,GST-π were detected by S-P immunochemiohistology. The patients were divided into two groups they were H Pylori positive group including 87 cases, H Pylori negative group including 51 cases. The data were analyzed to assess statistical differences with X^2 test. Results: There were 87 cases with H Pylori positive in this study,the positive rate of GST-π expression was 32.18% in the H Pylori positive group,which was significantly lower than that in H Pylori negative group(58. 82 %, X^2=8. 3, P〈0. 005), the GST-π expression rate in H Pylori positive two subgroups was lower significantly than that of two H Pylori negative subgroups and in either of two chronic gastritis groups too the average GST-π expres-sion rate in chronic atrophic gastritis mucosa was higher than that in chronic superficial gastritis but without statistical significance(38.54% vs 50% ,X^2 = 1.14, P〉0.25) the H Pylori positive rates of the two groups chronic superficial gastritis group and chronic atrophic gastritis group,were 63.54% and 61.91% respectively and there was no significant difference(X^2 = -0. 0006 ,P〉0.05). Conclusion: Expression of GST-π was significant lower in H Pylori positive than that in H Pylori negative the findings indicate that continous H Pylori infection may decrease expression of GST-π,and impair defensive function of GST-π.
