Application and evaluation of laparoscopic total mesorectal excision

腹腔镜直肠系膜全切除(TME)已在世界范围内获得较为广泛的开展。该文总结了腹腔镜TME在直肠癌保肛手术中的应用,包括手术原则、近远期疗效、生命质量等,旨在探讨目前TME术是仅停留在安全性与可行性的研究层面,还是已经成为“金标准”手术方式。然而,目前的循证医学证据仍停留在级别较低的非随机临床研究或缺乏对照的临床研究,还有待进一步的随机对照临床研究(RCT)证实腹腔镜TME术的远期疗效。

Laparoscopic total mesorectal excision（TME）has been widely used throughout the world. This review aims to analyse the application of laparoscopic TME in sphincter-preserving surgery for rectal cancer, including the surgical principle, short-term outcome, long-term prognosis and quality of life, and to answer the question whether laparoscopic sphincter-preserving TME is still just feasible or has served as the golden standard. However, recent evidence-based clinical studies of laparoscopic sphincter-preserving TME for rectal cancer are non- randomized or lack in controls. It is essential to conduct randomized controlled clinical trials to prove the potential long-term efficacy.
