有腹部开放性手术史的妇科腹腔镜手术技巧探讨

Laparoscopy technique in patient with past abdominal operation

目的:探讨有腹部开放性手术史的患者行腹腔镜手术的经验技巧。方法:回顾性分析237例有腹部开放性手术史患者的腹腔镜手术资料。结果:237例中235例成功完成手术,其中1例因局部粘连重损伤膀胱,1例因膀胱腹膜返折处血肿中转开腹。结论:有腹部开放性手术史并非是妇科腹腔镜手术的禁忌症,合理选择病人,熟练掌握腹腔镜操作步骤及技巧是顺利完成该类手术的关键。

Objective： To summarize the experience of laparoscopy in gynecology patients with past abdominal operation. Methods： Retrospectively review laparoscopy operation information of 237 cases with past abdominal operation. Results： In the 237 gynecology patients with past abdominal operation , 235 accomplished with lapaoscopy unevenfully, 2 cases were changed to open laparotomy because of the injury of bladder , and the other injury of the peritoneum and urocyst sturn back. Conclusion： Patients with past abdominal operation can benefit from lapaoscopy. Right steps and skills are keys to successful operations.
