随机扩增多态性DNA分析机械通气性肺炎大肠杆菌的多样性

摘要 目的:了解引起机械通气性肺炎大肠杆菌的主要基因型,探讨大肠杆菌的耐药机制。方法:运用随机引物扩增PCR技术对20株大肠杆菌进行指纹图谱分析,并采用双纸片确证实验进行ESBLs检测。结果:20株大肠杆菌经RAPD分为10个基因型,ESBLs阳性率达65.0%。20株大肠杆菌中17株在1447bp有共同条带。结论:机械通气性肺炎大肠杆菌感染基因具有多样性,患者主要为内源性医院内获得性感染,ES-BLs阳性检出率高,应引起临床的重视。 Objective To understand the main genetypes of Escherichia coli in mechanical ventilated pneumonia and to explore the mechanisms for drug resistance. Methods RAPD-PCR was used to analyze the fingerprinting of 20 strains of Escherichia coli. ESBLs were detected by double disk test. Results Twenty clinical strains of Escherichial coli were divided into 10 types by RAPD. The positive rate of ESBLs was 65.0%. Seventeen strains had the same line in 1 447 bp. Conclusion The genetype of Escherichia coli in mechanical ventilated pneumonia is diversified. Patients are mainly infected in hospitals. The detection rate of ESBLs is high, which should be paid much attention to.
