肾母细胞瘤螺旋CT征象与病理、微血管密度及血管内皮生长因子表达的关系 被引量:5

摘要 目的探讨肾母细胞瘤浸润转移螺旋CT(SCT)征象与病理、微血管密度(MVD)及血管内皮生长因子(VEGF)表达间的关系。方法采用免疫组织化学过氧化物酶标记的链霉卵白素法(SP法)检测38例术前行SCT检查的肾母细胞瘤组织标本中VEGF的表达,并计数MVD值。结果①SCT征象上邻近结构有无侵犯、淋巴结有无转移,VEGF的阳性表达率及MVD值差异均有统计学意义(P均<0.05)。②肾母细胞瘤高分期(Ⅲ+Ⅳ期)者的VEGF阳性表达率和MVD值均高于低分期(Ⅰ+Ⅱ期)者(P均<0.05)。③VEGF表达与肾母细胞瘤组织病理分型无关;MVD值与肾母细胞瘤的组织病理分型有关(P<0.05)。④VEGF阳性组MVD值高于VEGF阴性组(P<0.05)。结论肾母细胞瘤SCT征象与MVD、VEGF的表达有关,是肾母细胞瘤治疗前一项颇有价值的诊断技术,可用于预测肾母细胞瘤的侵袭和转移。 Aim: To investigate the correlation between Spiral CT (SCT) features and pathology, microvessel density (MVD) , and expression of VEGF in nephroblastoma tissue. Methods:Thirty-eight patients with nephroblastoma underwent SCT examinations. Microvessels and expressions of VEGF were examined using SP techniques and MVD was calculated. Results: In the infiltration group and the non-infiltration group and in the group with lymph node metastasis and that without, the positive rate of VEGF expression and MVD had significant difference (P 〈 0.05 ). The positive rates of VEGF expression and MVD in stage Ⅲ+Ⅳ were higher than those in stage Ⅰ+Ⅱ (P 〈 0.05). The positive rate of VEGF expression was not related with pathological types, while MVD was related with pathological types. MVD in the VEGF positive group was higher than that in the VEGF negative group (P 〈 0.05). Conclusion: Some SCT features,such as infiltration and lymph node metastasis, are closely correlated with MVD and expressions of VEGF in nephroblastoma tissue, and could be used in predicting aggressiveness and metastasis of nephroblastoma.
