风湿性心脏病围术期血浆降钙素基因相关肽含量变化及意义

The change and significance of calcitonin gene related peptide at perioperative period in patients with rheumatic heat disease

摘要 目的探讨血浆降钙素基因相关肽(calcitonin gene related peptide,CGRP)含量在风湿性心脏病(rheumatic heat disease,RHD)患者围术期变化及意义。方法采用放免法测定60例RHD患者围术期的血浆CGRP水平,与非手术组21例和健康对照组20例比较。结果(1)手术组术前血浆CGRP水平为(93.4±17.2)pg/ml显著低于对照组(106.4±20.1)pg/ml,而与非手术组差异无显著性,(2)术后第4 h明显升高[(356.6±40.1)pg/ml,P<0.01],以后维持高水平,与非手术组及术前比较差异非常显著(P<0.01)。结论RHD患者围术期CGRP明显增高,提示CGRP在RHD的病理生理改变中有重要意义,其血浆水平可作为判断RHD患者心功能和手术后心功能改善的一个指标。 Objective To investigate the change and significance of calcitonin gene related pepfide(CGRP) content during perioperative period in patients with rheumatic heart disease (RHD). Methods Rabio-immunity method was used to determine the serum levels of CGRP at perioperative period in operation group ( n = 60), and which were compared with those in non-operation group ( n = 21 ) and in control group ( n = 20). Results ( 1 ) Compared with those (106.4 ± 20.1 )pg/ml in control group, preoperative serum levels of CGRP in operation group (93.4± 17.2)pg/ml decreased significantly. Compared with those in non-operation group, there was no significant difference, (2) Serum levels of CGRP in operation group(4 hours after operation)increased significantly (356.6 ± 40.1)pg/ml, ( P 〈 0.01) and maintained the levels. Compared with those in control group and pre-operation group, there was a significant difference ( P 〈 0.01 ). Conclusion CGRP might be increased significantly during preoperation and pest-operation period in patients with rheumatic heart disease. CGRP may play an important role in pathophysiologic change of rheumatic heart disease. It's sertun levels can be a guidehne to estimate heart function of patients with RHD and heart function improvement after operation.
