西宁地区慢性阻塞性肺病加重期病人临床营养支持治疗

The clinical significance of nutritional support in patients with an aggravating stage of chronic obstructive pulmonary diseases in Xining Area

我院重症监护病房 (ICU) 1992～ 1995年对 2 6例慢性阻塞性肺病加重期病人进行TPN支持治疗并与 1992年前 30例进行比较 ,尽管两组病情基本一致 ,APACHEⅡ评分P >0 .0 5,但1992年后经TPN治疗组的病情明显好转。慢性阻塞性肺病加重期病人用TPN治疗有以下表现 :①呼吸肌力增加 ,肺自主通气量和潮气量增加 ;②脱离呼吸机时间与 1992年前组相比较均提前 3～ 5d ;③血清白蛋白 1周内均恢复正常 ,有利于损伤的肺泡和支气管上皮的修复 ;④体内各种电介质维持稳定 ;⑤医源性肺感染及其并发症减少 。

We treated 26 patients in aggravating stage of COPD by TPN from 1992 to 1995.Compared with 30 patients with the same illness not supported by TPN, although the seriousness of disease was the same with Apache Ⅱ score,the condition of TPN supported patients was alleviated greatly. The patients in aggravating stage of COPD treated with TPN had the increment in respiratory muscle strength and the leves of serum albumin,and the decreasee in the time of dislocation from respiratory machine and the rate of the complication of pulmonary infection.
