摘要 目的四肢骨折后进行脉搏血氧饱和度监测,判断创伤后远端肢体的血供情况,预测骨筋膜室综合征的发生。方法通过对129例四肢骨折患者,采用血氧饱和度仪动态监测肢端血氧饱和度(SpO2)来反映肢端的血供,对骨筋膜室综合征作出判断和预测及处理。结果3例小夹板过紧引起远端血供差;1例股骨骨折并股动脉栓塞;2例并发早期骨筋膜室综合征行手术减压,2例预后较好,1例预后差。结论四肢骨折后临床应用血氧饱和度仪动态监测肢端SpO2方法简便直观,可作为一个粗略、间接指标来对肢体是否出现血液供应不足,并发骨筋膜室综合征早期作出初步诊断。对治疗有一定的指导作用,尤其对临床观察有极大的帮助。 Objective The saturation carrying out pulse blood oxygen the day afer tomorrow monitors the four limbs fracture, occurrence that the room judging that blood of wound queen far end limb provides condition, forecasts bone anadesma collects synthetically.Methods Through to 129 example four limbs bone fracture patient, uses the blood oxygen degree of saturation meter dynamic monitor extremity blood oxygen degree of saturation (SpO2) to reflect the extremity the blood supplies, drafts to the bone fascia room makes the judgment and the forecast and processing. Results 3 example small clamping planks excessively are tight cause the far-end blood to siJpply the difference; 1 example thighbone bone fracture and femoral artery embolism; 2 example concurrent early time bone fascia room synthesis drafts the good surgery reduced pressure, 2 example prognoses are good, 1 example prognosis is bad.Conclusion After four limbs bone fracture clinical practice blood oxygen degree of saturation meter dynamic monitor extremity SpO2 method simple direct-viewing, may take sketchy, the indirect target comes to the body whether appears the blood insufficient supply, the concurrent bone fascia room synthesis drafts the early time to make the preliminary diagnosis. Has certain instruction function to the treatment, especially has the enormous help to the clinical observation.
