摘要
目的应用定量组织速度成像(QTVI)技术,对冠心病患者经皮腔内冠状动脉成形(PTCA)同时行冠状动脉内支架置入术治疗后效果的评价。方法15例冠心病患者,单纯左前降支狭窄,狭窄程度均≥75%,行PTCA和支架置入治疗后采用QTVI评价。分别于治疗前1天、治疗后1周及治疗后1月测量左前降支供血室壁各节段心肌收缩期峰值速度(PSMV)、舒张早期峰值速度(PDMV),比较治疗前后上述参数的变化。结果同治疗前相比,在左前降支供血区内治疗后1周各节段心肌PSMV、PDMV明显增快。治疗后1月,PSMV、PDMV增快更显著。结论QTVI技术可以定量分析冠心病治疗前后左室壁局部运动速度的变化情况,判断冠心病患者PTCA和支架置入治疗术后室壁运动的改善情况。
Objective To investigate the outcome of percutaneous transluminal coronary angioplasty ( PTCA} and stent implantation in coronary heart disease with quantitative tissue velocity imaging( QTVI). Methods Fifteen patients with isolated left anterior descending coronary artery (LAD) stenosis ( ≥75 % ) were included. QTVI was performed one day before PTCA + stent, a week and a month after successful PTCA + stent. Peak systolic myocardial velocity (PSMV) and peak early diastolic myocardial velocity (PDMV) were measured by QTVI. All parameters were measured in longitudinal direction ( basal, medial and apical segments) of septal, lateral, anterior, inferior, anteroseptal and posterior walls. Results In myocardium assigned by LAD, all regions showed a significant improvement of PSMV and PDMV a week after PTCA cantly a month after PTCA + stent. Conclusion The improvement of walls motion can be evaluated by QTVI in + stent, the above velocity parameters increased significhange of myocardial velocity can be quantified and the coronary heart disease after PTCA + stent.
