摘要 目的探讨儿童脑室周围白质软化症(PVL)的临床以及CT、MRI表现特征。方法回顾性分析33例儿童脑室周围白质软化症的临床资料和CT、MRI表现。结果临床表现:肢体瘫痪22例,智力低下8例,皮质盲3例,癫痫7例。CT/MR表现为:侧脑室三角区、体部周围、半卵圆中心脑白质密度/信号异常30例,上述区域白质减少26例,侧脑室扩大19例,侧脑室形态异常21例,胼胝体异常11例,侧裂池加深增宽17例。18例行MR检查,根据MR表现将PVL分为轻、中、重3度,本组属于轻、中、重度各6、8、4例。结论CT/MR可清晰显示PVL的病变程度、范围,MR显示病变更敏感,临床表现与MR所见密切相关。 Objective To study the characteristics of clinical and CT/MR/fmdings of periventricular leucomalacia(PVL) in children. Methods The clinical and CT/MRI findings of 33 cases of PVL were retrospectively analyzed. Results Clinical findings: paralysis in 22 cases, mental retardation in 8 cases, cortical blindness in 3 cases, epilepsy in 7 cases. The main CT and MR/findings were: density/signal intensity abnormalities were found in white matter of trigone ,periventricular body and semiovale center ( n = 30) ; quantitative reduction of white matter in above areas ( n=26) ; enlargement( n = 19) and irregularity( n = 21 ) of the lateral ventricles; abnormal corpus callosum ( n = 11 ) ; lateral fissure deepen and widen ( n = 17 ). 18 cases of PVL who performed MR seans were classified into three degrees based on MR/ appearances , mild(n=6), middle(n=8), severe(n=4). Condusions ThePVL can been clearly displayed by CT/MR. MRI is more sensitive than CT scan, and clinical manifestation were closely related to the findings of MRI.
