摘要
目的探讨功能性消化不良(Functional dyspepsia,FD)患者脂肪餐前后血浆胆囊收缩素(Cholecystokinin,CCK)水平的变化及其在FD发病中的可能作用。方法对21例FD患者与10例健康对照者用放免法检测其脂肪餐前后血浆胆囊收缩素水平的变化。结果对照组脂肪餐后0.5h,1.0h,1.5h血浆CCK水平较空腹显著增高(P<0.05);FD组脂肪餐后1.0h,1.5hCCK水平较空腹明显升高(P<0.05);对照组血浆CCK水平在脂肪餐后呈直线上升趋势,在餐后1.0至1.5小时达到高峰,并且开始下降;而FD组脂肪餐后呈缓慢上升趋势,在餐后0.5至1.0小时CCK水平开始上升,并在1.0至1.5小时仍持续一定水平不下降;FD组的空腹CCK水平较对照组显著增高(P<0.05)。结论功能性消化不良患者空腹血浆CCK分泌增多,并存在血浆CCK脂肪餐后分泌曲线的异常。CCK水平变化可能与功能性消化不良发生机制有关。
Objective To detect the change of the level of plasma CCK before and after fatty diet test, and to illuminate the possible mechanism of dyspepsia in patients with functional dyspepsia (FD). Methods In FD group, the 21 patients all met with Rome Ⅱ diagnostic criteria of FD. 10 healthy patients were included in group N as control . After fatty - meal test , the plasma CCK at Oh, 0.5h, lh and 1.5h was assayed respectively by the way of radioimmunoassay. Results IngroupN, thelevelofCCKat0.5h,1h and 1. 5h after fatty - meal test was significantly higher than that in 0h after fatty - meal test( P 〈 0.05). In group FD the level of CCK at lh and 1.5h after fatty - meal test was significantly higher than that at Oh after fatty - meal test( P 〈 0.05).The level of CCK pre- meal in group FD was significantly higher than that in group N (P 〈 0.05), meanwhile there was no significant difference among the groups after fatty - meal test at 0.5h, lh and 1.5h respectively (P 〉 0.05). The curve of CCK in group N reached the peak level at 1 .Oh and dropped down within 1.5h. Meanwhile, the level of CCK in group FD raised slowly even after 1.5 hours by fatty - meal test. Conclusions The levels of plasma CCK pre - meal is raised in FD patients . And the curve of secretion of CCK after fatty - meal is abnormal. Pine change of secretion of CCK may play a role on dyspepsia.
