湖南汉族强直性脊柱炎与HLA-B27、B7、B13和B40抗原的关联性分析

Analysis of the relationship between ankylosing spondylitis and HLA-B27, B7, B13 as well as B40 in Hunan Han nationality

目的探讨湖南汉族疑似强直性脊柱炎患者的HLA-B27、B7、B13和B40抗原分布及其关联性。方法采用补体依赖性微量淋巴细胞毒法(NIH标准)检测HLA-B27等相关B位点抗原。结果①在450例患者中,男性患者的HLA-B27、B7抗原阳性率明显高于女性患者,而B13、B40抗原阳性率无性别差异;②患者组HLA-B27、B7阳性率均高于对照组(RR=13.92、RR=8.21),而B13阳性率低于对照组(RR=0.21),B40在两组的阳性率高且分布无差异。③患者组B7/B40的抗原阳性率高于对照组(χ2=19.5,P<0.0001),RR=22.64;患者组B13/B40的抗原阳性率低于对照组(χ2=20.57,P<0.0001),RR=0.27。结论强直性脊柱炎与B27和/或B7有关联。

[Objective] To investigate the phenotype and relationship of HLA-B27, B7, B13 as well as B40 loci in Hunan Han nationality suffering Ankylosing Spondylitis. [Methods] A complement dependent lymphocytotoxicity assay （NIH criterion） was adopted to detect the HLA-B27 and related B locus. [Results] ①In a group of 450 pa- tients, the positive rates of HLA-B27 and B7 were significantly higher in males than that in females. Whereas there were no gender difference of HLA-B13 and B40. ②The positive rates of HLA-B27 and B7 were significantly higher （RR=13.92, RR=8.21） but lower for HLA-B13 （RR=0.21） in the patients group than that in the control group, whereas B40 of no difference between the two groups. ③The positive rates of B7/B40 were higher （RR=22.64） but lower for B13/B40 （RR=0.27） in the patients group than that in the control group. [Conclusion] Ankylosing spondylitis is closely related to the HLA-B27 and/or B7.
