半月板旋转型人工膝关节置换治疗中老年膝关节病 被引量:1

Rotating-platform total knee arthroplasty in the treatment of the knee arthropathy in wrinkly and old folks

摘要 目的探讨半月板旋转型人工膝关节置换(rotating-platform total knee arthroplasty)治疗中老年膝关节病的临床特点、手术技巧和并发症防治。方法回顾性分析该院2002年1月~2006年1月施行的半月板旋转型人工膝关节置换42例54膝,采用HSS(hospitalforspecialsurgery,HSS)膝关节评分标准和美国膝关节协会全膝关节置换术X线评分进行手术前后比较。结果42例术后随访6~49个月,平均21个月,HSS评分术后平均为93.1分,采用统计软件SPSS11.5分析,t=-45.35,P<0.01,优良率达100%。活动范围术后平均129.4°,X线评分提高到术后平均90.98分,t=-31.71,P<0.01。全部患者均可站立及行走,未发现假体松动。结论半月板旋转型人工膝关节置换术后无痛、稳定、活动好,手术操作简便,并发症少,近期效果满意。 [Objective] To evaluate the clinical features, the operative technique and the methods of preventing complication of the treatment of knee arthropathy in wrinkly and old folks with rotating-platform total knee arthroplasty. [Methods] From January 2002 to January 2006, We retrospectively analyzed the clinical data of the 42 patients (54 knee) and performed the operation of rotating-platform total knee arthroplasty. The clinical results of afteroperation and pre-operation were evaluated by the HSS knee rating score system and score system of X radiographs which was constituted by American knee association. [Result] The 42 patients (54 knees) were followed-up for the average time of 21 months (form 6 months to 49 months). The average HSS knee rating score improved to 93.1, and the good-to-excellent clinical result was 100%. SPSS (virsion 11.5) statistical software was used for analysis, t =- 45.35, P 〈0.01. The average range of motion was improved to 129.4 degrees, and the average score of X radiographs improved to 91, t =-23.71, P 〈0.01. All patients had walking and standing ability after-operatively. All patients could revert the function of standing and walking after operation without any plate loosing. [ Conclusions] The painless, stabilization, and well- function knee could be obtained after the operation of rotating-platform total knee arthroplasty. We will be satisfied with operative outcomes in the near future with easily surgical operation and less complications.
