先天性心脏病介入治疗285例疗效评价

Clinic assessment of interventions in 285 congenital heart disease cases

目的探讨先天性心脏病介入治疗的临床疗效。方法回顾分析该院2003年6月￣2005年4月连续施行的先天性心脏病介入治疗285例,其中动脉导管未闭(PDA)封堵术87例,房间隔缺损(ASD)封堵术104例,室间隔缺损(VSD)封堵术75例,肺动脉瓣狭窄球囊扩张术(PBPV)15例,冠状动脉瘘栓塞术4例;采用经皮穿刺股静脉和/或股动脉法,术前、术后行左、右心导管造影检查。结果技术成功率97.54%(278/285),并发症2.81%(8/285),紧急手术0.70%(2/285),死亡率0.35%(1/285)。结论先天性心脏病介入治疗具有成功率高、创伤小、并发症及死亡率低等特点,对大部分病例可替代外科手术。

[Objective] To evaluate the results of interventions in patients with congenital heart diseases （CHD）. [Methods] The clinal effects of interventional therapy in consecutive 285 patients with CHD at Xiangya hospital from June 2003 to April 2005 were retrospectively analyzed, and the patient distribution included patent ductus arteriosus （PDA） 87, atrial septal defect （ASD） 104, ventricular septal defect （VSD） 75, pulmonary stenosis 15, and cornnay artery fistula 4. All the interventions were performed via puncture of femoral artery and/or vein, and right and left cardiac angiography were conducted on all of the patients before and after the procedure. [Results] The technical successful rate was 97.5%（278/285）, complication rate was 2.8%（8/285）, and emergency surgery was done in 2 patients 0.7% （2/285）. The procedure- related mortality rate was 0.35%（1/285）. [Conclusion] Interventional therapy of congenital heart disease is characterized with a high successful rate, less invasive, low complication and mortality rate, and it has become promising alternative therapy to surgery in most patients.
