对侧腹股沟皮瓣修复单个手指皮肤套脱伤

Contralateral groin skin flap in the treatment of single digital degloving-injury

目的探讨对侧腹股沟皮瓣修复单个手指皮肤套脱伤的治疗效果。方法对各种外伤引起的不同程度的单个手指皮肤套脱伤的患者共46例,均行急诊手术清创,并采用对侧腹股沟皮瓣转移修复,于3、4周后行二期手术断蒂。结果46例患者术后当天皮瓣血运均良好。其中3例术后第2天出现皮瓣下血肿并导致皮瓣远端局部坏死,经再次手术清除血肿并去除远端坏死皮瓣后,仍用原皮瓣转移修复,不影响手术效果。二期手术断蒂后手指保留了原来长度且功能基本恢复,患者基本感到满意。结论采用对侧腹股沟皮瓣转移修复单个手指皮肤套脱伤,是一种比较可行的方法。

[Objective] To explore the treatment of single digital degloving-injury with contralateral groin skin flap. [Methods] 46 patients with single digital degloving-injury were admitted and operations were peformed emergently. The injured digit was treated with the contralateral groin skin flap. The pedicle of flap was cutted off 3 to 4 weeks after the first operation. [Results] All the flaps survived except that 3 patients were re--operated on the second postoperation day because of the under-flap hemorrhage and the secondary necrosis of the distal part of the flaps. All the injured digits were reconstructed without reducing the length of them and the patients were satisfied with the operations. [Conclusion] The eontralateral groin skin flap is a preferable choice for reconstructing the single digital degloving-injury.
